RECEIVING SMART CARD APPLICATIONS

A collection point for receiving smart card applications of drivers or companies You can pick up the application/form/postal order with a unique variable symbol during MON-FRI from 7.00am until 3.30pm.

For security reasons, the driver card application must be submitted by the driver in person and the company card application must be submitted by the company owner or a person authorised by him/her in person. Notice! The authorisation must be duly certified by a notary.

As regards the driver card application, the following is required:

1. ID card 2. Driver's licence 3. If the card is renewed, bring your existing driver card as well 4. If the driver is not a citizen of the Slovak Republic, he/she is obliged to submit a document proving the approval of residence in the Slovak Republic

As regards the company card application, the following is required:

1. A legibly written application according to the instructions on the back of the application 2. ID card 3. If the card is renewed, bring your existing card as well

After sending the application, the card will be sent to your address by post within 30 days.
